Namaz e Mayyat
The Deceased Prayer (Salat al-Mayyit) consists of five Takbirs (saying Allahu Akbar). After each Takbir, a specific supplication is recited as follows:

Text and Translation
After the first Takbir, say:
اَشْهَدُ اَنْ لَّاۤ اِلٰهَ اِلَّا اللّٰهُ وَ اَشْهَدُ اَنَّ مُحَمَّدًا رَّسُوْلُ اللّٰهِ
ASH-HADU AL LAA ILAAHA ILLALLAAHU WA ASH-HADU ANNA MUHAMMADAR RASOOLULLAAH
I bear witness that there is no god save Allah, and I bear witness that Muhammad is the Messenger of Allah.
After the second Takbir, say the Salawat:
اَللّٰهُمَّ صَلِّ عَلٰى مُحَمَّدٍ وَّ اٰلِ مُحَمَّدٍ
ALLAAHUMMA SALLI A'LAA MUHAMMADIN WA AALI MUHAMMAD
O Allah, send blessings upon Muhammad and the Household of Muhammad.
After the third Takbir, pray for the faithful believers:
اَللّٰهُمَّ اغْفِرْ لِلْمُؤْمِنِيْنَ وَ الْمُؤْمِنَاتِ
ALLAAHUMMAGH-FIR LIL-MOO'MINEENA WAL MOO'MINAAT
O Allah, forgive the believing men and the believing women.
After the fourth Takbir, pray for the deceased:
اَللّٰهُمَّ اغْفِرْ لِهٰذَا الْمَيِّتِ
ALLAAHUMMAGH-FIR LI-HAADHAL MAYYIT
O Allah, forgive this deceased person.
Then recite the fifth Takbir and conclude the prayer.
