Sayyid Ibn Tawus, in his book of Misbah al-Za'ir, has mentioned this supplicatory prayer to be said directly after the comprehensive form of ziyarah that is known as Ziyarah of the Imams of the Believers:
Text and Translation
بِسْمِ اللّٰهِ الرَّحْمٰنِ الرَّحِيْمِ
BISMILLAAHIR RAHMAANIR RAHEEM
In the Name of Allah, the All-beneficent, the All-merciful.
اَللّٰهُمَّ اِنِّيْ زُرْتُ هٰذَا الْاِمَامَ مُقِرًّا بِاِمَامَتِهٖ مُعْتَقِدًا لِفَرْضِ طَاعَتِهٖ
ALLAAHUMMA INNEE ZURTU HAADHAL IMAAMA MUQIRRAM BI-IMAAMATIH, MU'TAQIDAL LI-FARDI TAA'ATIH
O Allah, I am visiting this Imam confessing his Imamate and believing in the obligation of obedience to him;
فَقَصَدْتُ مَشْهَدَهٗ بِذُنُوْبِيْ وَ مُوْبِقَاتِ اٰثَامِيْ وَ كَثْرَةِ عُيُوْبِيْ
FAQASADTU MASH-HADAHU BI-DHUNOOBOOBEE WA MOOBIQATI AATHAAMEE WA KATHRATI U'YOOBEE
therefore, I have betaken myself to his shrine in spite of all of my sins, mortal offenses, and numerous defects,
وَ شِدَّةِ وَزَرِيْ وَ اَنَا مُسْتَجِيْرٌ بِكَ فِيْ هٰذَا الْوَقْتِ وَ بِهٖ
WA SHIDDATI WAZAREE WA ANAA MUSTAJEERUM BIKA FEE HAADHAL WAQTI WA BIH
and the heaviness of my burdens, and I am seeking refuge with You at this time and through him,
فَاغْفِرْ لِيْ ذُنُوْبِيْ كُلَّهَا يَا اَرْحَمَ الرَّاحِمِيْنَ
FAGHFIR LEE DHUNOOBOOBEE KULLAHAA YAA ARHAMAR RAAHIMEEN
so forgive me all my sins, O most Merciful of all those who show mercy!
